Instructions & Information:
To be eligible for a wholesale account with BioLongevity Labs, you must be a wellness practitioner, wellness clinic, or a provider. Where applicable, a business license and/or medical registration is required.

Ensure you provide a valid email address so we can notify you once your account has been approved and activated.
All wholesale orders must meet a minimum purchase requirement of $5,000.
Please email us a copy of your Sales Tax Exemption form.
Online sellers:
BioLongevity Labs is the only authorized retailer for our products on third-party marketplaces. Online sellers are only authorized to sell BioLongevity Labs products on their own websites unless written approval is granted by BioLongevity Labs.
